Forms allows you to create your own set of questions to send to your users and get submission right into your Discord server! This module also allows further Discord integrations, such as allowing you to do ban/mute appeals.
Permissions Check
- Make sure Dyno has these permissions in your server:

- Head over to your account and select your server.
- Click on the
Modules tab.
- Ensure the
Forms module is enabled
- Click the
Settings button underneat the Forms module.
- Click
Create Form, and type in a form name and description (supports markdown).
- Under
Form Builder, you can add, edit, and delete questions for your form. You can also click Add Template to start off with a template which will give you premade questions based on what kind of form you’re making. (ex. Staff Applications, Appeals, or Feedback Survey).
- To add a question, click the
Add Question button, and then click the question name or its answer field. You can then begin editing the question name, choose what kind of question it is (paragraphs, short answers, multiple choice, checkboxes, and more coming soon!) and rather to require an answer for this question and/or duplicate the question.
- Under the last question, you can also clear all questions, or undo your last action (if you made a mistake!).
- Under
Form Settings, select a Submissions Channel. Once a user makes a submission, Dyno will send an embed here with all the form questions and answers, along with the user info (if the form isn’t anonymous).
- Under
Form Settings, Options, and Submission Options, there are other settings you can change. Click the “?” next to each one to learn more about what it does. There are also in-depth explanations of each one below.
- Click
Create, then you’re done! Click View to view the form and share the link with your users to get submissions going!

- Non-Premium servers can have a max of 3 forms.

There are two ways to view submissions:
- Only server admins/owner can do this, and that’s by going to the server dashboard, and finding the form that you created, there is a button to view submissions.
- Submission managers/viewers can view submissions using this method. Visit the form page (ex. dyno.gg/form/[formID]), and on the bottom (next to the submit button), you will see a
View Submissions button. From there on, you can view the list of submissions.
- Submissions Channel | The channel where Dyno will post the submissions.
- Submissions Viewers | Members with these roles will be able to view full submissions (through the dashboard). These members cannot perform any actions on any submissions.
- Submissions Managers | Members with these roles will be able to view and manage form submissions in the submission channel and on the dashboard.
- Mute/Ban Appeals | Convert this form into an Appeals form. You can then manage your Appeals settings on the Appeals page under the Moderation module settings. This option must remain enabled for as long as the form is selected as the Appeals form.
- Allowed/Ignored Roles | Roles added here will/will not be able to use the form, respectively. You can only select roles in one of the options, as if you select an Allowed Role, Dyno will automatically disable all other roles, and vice versa.
- Members Only | Only users that are members of your server will be able to fill out the form. You can also optionally attach a server invite in the form for users that are not currently in your server.
- Anonymous Submissions | Submission Managers will not be able to see user information on submissions. Logging in with Discord to fill out the form is still required for anti-abuse purposes.
- One Submission Per User | Users will only be allowed to fill out the form once.
- Enforce Submission Cooldown | Allows you to select a submission cooldown, which will limit users to filling out the form once every
x days.
- Create Thread On Submission | Dyno will create a thread in the submission channel when the submission message is posted.

- Mention @everyone / @here / @roles On Submission | Roles you select here will be mentioned in the submission message whenever someone submits the form. You can mention up to 10 roles.
- Add Reactions To Submission Message | Allows you to select reactions that Dyno will automatically add to the submission message whenever someone submits the form.
- Add Roles To Submitter | Dyno will add the role(s) specified when a user creates a submission.
- Remove Roles From Submitter | Dyno will remove the role(s) specified when a user creates a submission.
You can only add or remove up to 5 roles in total.
